With this form I have enclosed the Fee of $20.00/hour for the "FAMILY TREE RESEARCH" genealogy search.

1)  I would like to pay for research hours in the amount of  (The more hours you give us to research, the better the chances of finding valuable data on your ancestors.  If you want information on only one ancestor, just two hours may suffice.):

I understand that this "FAMILY TREE RESEARCH" service cannot and does not guarantee specific findings in advance.  It simply is not possible to know what may be found until after the research has been conducted on my ancestor(s).
